Michael Sullivan, LPC, CAAC, NCC is a fully licensed professional counselor in the state of Michigan. This license (LPC) permits professional counselors to conduct psycho-therapeutic interventions and assessments for individuals, groups, and families. Michael is also certified as an Advanced Addictions Counselor (CAAC) by the Michigan Certification Board for Addiction Professionals and the International Certification and Reciprocity Consortium. Michael is a nationally certified counselor and is a member of the Michigan Counseling Association and the American Counseling Association.

Instructions for FIRST TIME clients:

1. Read and sign DISCLOSURE STATEMENT, bring to appointment

2. Please fill out the CLIENT INTAKE FORM appropriately.

3. Bring completed forms, accompanied by initial 1st appointment fee, $90. Bear River Health accepts check, money order and cash, due at time of service.
